Your hip pain started gradually—a bit of stiffness in the morning, occasional groin discomfort, difficulty getting comfortable at night. Now it's limiting your walking, affecting your work, and making simple movements like putting on shoes genuinely difficult.
You've done the stretches, strengthened your glutes, and worked on your hip mobility. But the deep ache persists, and certain movements still create sharp catching pain that makes you hesitate.
Chronic hip pain that won't resolve despite exercise isn't about lacking commitment—it's about neuromuscular control patterns and compensation strategies that 30-minute physio appointments can't adequately address.
I'm Tim Eslick, an Exercise Physiologist specialising in chronic hip pain treatment in Edinburgh. I use Dynamic Neuromuscular Stabilisation (DNS) therapy to identify and treat the root causes of persistent hip dysfunction.
Why Your Hip Pain Isn't Improving
You've probably heard the standard advice:
Stretch your hip flexors
Strengthen your glutes
Work on single-leg balance
Improve your hip mobility
These work for simple hip tightness. But chronic hip pain that persists despite treatment indicates deeper dysfunction.
The Hip Mobility Paradox
Here's what confuses most people:
Your hip might test with good range of motion on the physio table—you can move it through full flexion, extension, abduction. But during functional movements like walking, climbing stairs, or standing from sitting, your hip doesn't move properly.
This is the difference between passive mobility and functional control.
Your hip has the range of motion in isolation but lacks the neuromuscular control to use that range under load. Your nervous system has developed compensation patterns—using your lower back, relying on momentum, or avoiding certain movement angles—to protect the hip.
Standard exercises strengthen muscles but don't necessarily restore the integrated control patterns your hip needs for pain-free function.
What 30-Minute Appointments Miss
Standard physio for hip pain typically addresses:
Hip flexor stretching (psoas, iliacus, rectus femoris)
Glute strengthening (clamshells, bridges, monster walks)
Single-leg balance exercises
Perhaps some manual therapy
This works for many acute hip issues.
What gets missed in short appointments:
Deep compensation patterns affecting your whole kinetic chain
How your hip interacts with your core stability and breathing
Fascial restrictions limiting natural hip mechanics
Subtle asymmetries in how you load each hip during movement
Whether your hip pain is actually coming from lumbar spine or SI joint dysfunction
These require extended assessment time and hands-on treatment to identify and address.
How DNS Therapy Treats Chronic Hip Pain Differently
Dynamic Neuromuscular Stabilisation addresses hip dysfunction by examining your complete movement system—not just the hip in isolation.
The DNS Approach to Hip Dysfunction
90-Minute Initial Assessment
Extended time allows thorough investigation:
Complete kinetic chain assessment - hip pain often stems from lower back dysfunction, pelvic asymmetry, or knee compensation patterns. We assess everything from your thoracic spine to your feet.
Functional movement testing - how does your hip perform during squatting, single-leg stance, rotational movements, and gait? Where does control break down?
Fascial restriction mapping - identifying tight hip flexors, restricted glutes, adhesions in your hip capsule, and IT band tension limiting movement.
Core and hip integration testing - can your core stabilise your pelvis while your hip moves? Does your breathing pattern affect hip mechanics?
Load tolerance assessment - when does pain appear during progressively challenging movements? This reveals the specific control deficit causing your pain.
Hands-On Fascial Release and Mobilisation
Chronic hip pain creates extensive fascial restrictions throughout your hip complex, lower back, and pelvic region.
DNS treatment includes:
Deep hip flexor (psoas, iliacus) fascial release
Glute and piriformis release
Hip capsule mobilisation
IT band and TFL fascial work
SI joint and lumbar spine mobilisation
Adductor and groin muscle release
Most clients notice immediate improvement in hip range of motion and reduced pain during the first session.
Neuromuscular Re-Education and Integration
This is where DNS differs from standard hip treatment:
Rather than isolated hip strengthening, DNS retrains the integrated patterns controlling hip function:
Pelvic stability during hip movement - can your pelvis stay stable while your hip moves through range?
Core integration - connecting hip control to proper breathing and core stability
Gait retraining - correcting asymmetrical loading patterns during walking
Load progression - building hip capacity under real-world demands
Common Chronic Hip Conditions I Treat in Edinburgh
Deep Hip/Groin Pain
Symptoms: Deep ache in front of hip or groin. Pain with walking, stairs, or getting in/out of car. Often worse after sitting and when first standing up.
Root causes: Hip flexor tightness, hip capsule restrictions, femoral acetabular impingement (FAI) mechanics, or referred pain from lumbar spine.
DNS approach: Differentiating true hip joint pain from referred lumbar pain. Extensive hip flexor and capsule work. Retraining hip control to reduce impingement mechanics.
Hip Pain After Walking/Running
Symptoms: Hip feels fine at rest but develops pain during or after walking/running. Often lateral hip pain or deep groin discomfort.
Root causes: Poor gait mechanics, weak hip stabilisers allowing excessive movement, or compensation for other areas (ankle, knee, lower back).
DNS approach: Detailed gait analysis. Identifying asymmetries and compensation patterns. Retraining proper hip loading during walking/running.
Lateral Hip Pain (Greater Trochanteric Pain Syndrome)
Symptoms: Pain on outside of hip, often worse lying on that side, climbing stairs, or standing on one leg. Previously called "trochanteric bursitis."
Root causes: Weak hip abductors (glute med/min), IT band tension, altered gait mechanics creating excessive compression.
DNS approach: Releasing IT band and TFL restrictions. Retraining glute activation and hip abductor endurance. Correcting gait patterns creating excessive compression.
Hip Stiffness and Limited Range of Motion
Symptoms: Hip feels locked or restricted. Difficulty putting on shoes/socks, getting in/out of low chairs, or moving freely during activities.
Root causes: Hip capsule restrictions, chronic muscle guarding, or fascial adhesions from prolonged dysfunction or previous injury.
DNS approach: Extensive capsule mobilisation and fascial release. Progressive movement restoration without forcing range.
Post-Physio Hip Pain That Plateaued
Symptoms: Completed physio with some improvement but hip still isn't right. Still limited in certain movements, residual pain, or fear of re-aggravating.
Root causes: Standard exercises improved strength but didn't restore functional neuromuscular control under load.
DNS approach: Testing hip stability under progressively challenging conditions. Identifying remaining control deficits that isolated exercises missed.
Why Choose Relief and Restore for Hip Pain in Edinburgh
DNS Specialisation: Advanced Prague School certification. DNS is particularly effective for hip dysfunction because it addresses the relationship between hip control, core stability, and whole-body movement patterns.
Extended Assessment Time: 90-minute initial appointments properly identify the neuromuscular control deficits and fascial restrictions that short appointments miss.
Gait Analysis Expertise: Detailed assessment of how you walk and load your hips during functional movement. Most hip pain involves gait dysfunction.

Common Questions From People Who've Tried Other Treatments
-
Most physio appointments are 30 minutes and focus on exercises. Our initial assessment is 90 minutes, allowing us to identify compensation patterns that shorter evaluations miss. We use hands-on neuromuscular techniques to treat root causes, not just manage symptoms.
-
We specialise in complex cases that haven't responded to standard treatment. Most clients see improvement within 4-6 sessions, though timelines vary. We'll honestly tell you during your free consultation or initial assessment if we think we can help.
-
It depends on how long you've had the pain and what's causing it. Typical ranges: 2-4 sessions for acute flare-ups, 6-8 for moderate chronic pain (3-12 months), 8-12 for long-standing issues. We'll give you a realistic estimate after your initial assessment.
-
About 2 metres by 1.5 metres—roughly the size of a yoga mat. Living rooms, bedrooms, or spare rooms work perfectly. We bring all equipment including a treatment mat.
-
None. We bring everything required. Just wear comfortable clothing you can move in (shorts/leggings and a t-shirt) and have water nearby.
-
Many clients feel some relief during the first session. Noticeable improvement typically comes within 1-2 sessions, with significant change by 4-6 sessions. Chronic pain takes time to resolve—we'll give you a realistic timeline after your assessment.
